LPN / RN at Title Wellness
About Title WellnessTitle Wellness is a BHRT and hormone optimization practice in Medfield, MA, offering hormone therapy, medical weight loss, peptide therapy, and IV therapy. Since opening, we've built a loyal patient base on personalized, physician-guided care. We're now scaling: adding provider capacity, expanding our patient portfolio, and building the operational backbone to support that growth. This role is one of several we're hiring for right now — each with its own distinct scope and reporting line, so you'll know exactly what you own from day one.
The Role
We're looking for an LPN or RN to own the clinical support side of our practice — everything medically related that doesn't require a provider's direct hands-on involvement, but does require licensed nursing judgment. You'll handle patient medical questions within your scope, administer injections, initiate and manage IV therapy, and check in with patients on how their weight-loss and BHRT treatments are progressing. Anything outside your scope gets triaged directly to the provider. This is not a front-desk or scheduling role — our Receptionist owns that lane, and you'll work alongside them.
What You'll Own
- Field and answer medically-related patient questions within your scope of practice, escalating to the provider when a question falls outside written protocols
- Administer injections (hormone therapy, peptides, and related treatments)
- Initiate and manage IV drip therapy — we'll support obtaining or completing any additional IV certification needed
- Perform blood draws as needed
- Conduct follow-up check-ins with weight-loss patients on progress, side effects, and adherence
- Conduct follow-up check-ins with BHRT patients on how their treatment is working and whether adjustments should be flagged to the provider
- Educate patients on their treatment plans, medications, and what to expect
- Follow written standing orders and protocols precisely, and recognize when a patient's situation falls outside those protocols and needs direct provider consultation
- Maintain accurate clinical documentation in Optimantra
What You Bring
- Active Massachusetts LPN or RN license in good standing
- Comfort administering injections and blood draws; IV certification a plus (or willingness to obtain it)
- Experience in a hormone therapy, med-spa, weight-loss, or specialty outpatient clinic setting preferred
- Strong clinical judgment about when to handle a patient concern directly versus when to escalate to the provider
- Comfortable following written protocols and standing orders precisely and documenting care thoroughly
- A patient-education mindset — you enjoy explaining treatments and answering questions in a way patients understand
Why This Role
This role gives you real clinical ownership — you're not doing intake paperwork or scheduling, you're the clinical touchpoint between our patients and our provider. As we grow, this role sits at the center of patient care quality and retention.
